St. Anne Faith Formation Program and Sacramental Preparation Requirements

1. I understand my child must be enrolled in 2 consecutive years of Faith Formation Classes for sacramental preparation. (First Communion/Confirmation).

2. I will been given key dates and agree to attend these. (meetings, mass, services and student retreat.

3. I understand that my child must regularly attend mass the 1st and 2nd year of Faith Formation classes when they're preparing for the sacraments.

4. I understand that my child may be dropped from Faith Formation classes for poor attendance. If my child misses 3 or more classes, a parent meeting may be required.

5. I agree to make up all assignments my child missed when they were absent.

6. I agree to submit the Parental Permission,Health Authorization, Release Form to the Parish Office before the first day of class.

7. I agree to submit the registration payments prior to the first day of class, through the online giving link below or by cash/check payment in the parish office.
